Request home adaptations and an occupational therapy assessment
When to use this
- You or someone in your home struggles to move around safely, wash, or use the stairs or toilet.
- You think you need changes like grab rails, a ramp, a stairlift or a walk-in shower.
- You have been in hospital, or are about to leave, and your home is hard to manage.
What to attach
- Any letter from your doctor, hospital or occupational therapist
- Photos of the areas that are hard to use, such as the stairs or bathroom

After you send it
Adaptations are usually arranged through your council's adult social care team, who can carry out an occupational therapy assessment and tell you about a Disabled Facilities Grant. If you rent and the problem is also a repair the landlord should fix, call us on 0800 030 4669.
