Dear [Landlord or Housing Officer name],

Re: Request for alternative accommodation, [Your address]

I am writing to ask you to provide alternative accommodation for myself [and my household: list who lives with you] while repairs are carried out at the above address [or: while a prohibition order / prohibition notice is in force].

My home is currently unfit to live in because: [describe clearly, for example: the roof is leaking into the main bedroom and living room / there is no heating and it is winter / a prohibition order has been served on the property / the ceiling has partially collapsed].

I have been [unable to use the bedroom / unable to use the [room] / living in dangerous conditions] since [date].

[If a prohibition order is in force: Your legal duty to provide alternative accommodation arises because the council has served [an emergency prohibition order / a prohibition order] on [date]. A copy is attached.]

[If repairs will require me to leave temporarily: The contractor / surveyor has told me that the repairs will require me to leave the property for approximately [time period].]

Please arrange suitable alternative accommodation for my household as soon as possible, and in any event within [3 working days / 24 hours if this is an emergency].

If you are unable to provide accommodation directly, I expect you to meet the reasonable cost of my staying elsewhere during the period required.

Please confirm in writing that you will arrange this and when.

Yours sincerely,
