Dear [Landlord or Housing Officer name],

Re: Request for reasonable adjustments at [your address]

I am disabled, or have a health condition, and I am asking you to make a reasonable adjustment under the Equality Act 2010 so I am not put at a disadvantage.

[Explain what you need. For example: please contact me by email rather than phone, please let me have a parking space nearer the door, or please make an exception to a rule that affects me because of my condition.]

Please tell me whether you can make this change, and if not, why not.

Yours sincerely,
