When to use this
- You are in hospital, or about to leave, and cannot return home safely until something is fixed or changed.
- A repair, hazard or lack of adaptation is delaying your discharge.

- Any letter from the hospital or ward about your discharge needs
- Photos of the problem if it is a repair or hazard

After you send it
Ask the hospital discharge team or your social worker to support your request, as they can often push for urgent action. If the problem is a repair your landlord must fix, call us on 0800 030 4669.
